2. One scanned copy of completed medical form for child by medical Doctor
3. One scanned copy of completed dental form for child by Dentist
4. Scanned copy of Bank vouchers stamped by Bank
5. One scanned copy of completed “DECLARATION OF LAST SCHOOL ATTENDED” form by Head Teacher
6. Scanned copy of signature for child, father, mother or guardian (as separate files)

FILE DOWNLOAD IN THE EVENT THE COPY OBTAINED FROM SCHOOL IS MISPLACED.
